Office Procedures

Office visits are done entirely over the internet via live interactive video coaching sessions.  This provides you with the convenience and privacy of being in your own home during coaching.

To provide you with the highest degree of security currently available, I have contracted with VSee.com, the most trusted HIPAA compliant telemedicine solution on the market.  Vsee.com was chosen by the National Aeronautics and Space Agency (NASA) for use by the astronauts on the space station.  Vsee.com, is also used by over a thousand other companies including Walmart, Walgreens and Shell.  To schedule an appointment, simply click the “schedule appointment” button on the home page of this website and follow the prompts.  You will be linked to the VSee sub-domain.  On your initial visit, click the “Sign Up” button on the top right corner of the page.  You will then be prompted to provide your email address, which will then be used to send instructions to you on how to set up an account.  This step is to ensure that you are not a robot. Follow the prompts to set up a password and then fill in the Registration Form and provide your payment information.  Be assured that your card will not be charged until after your appointment has been attended, which probably will be several days from now.  In scheduling an appointment keep in mind the available times indicated when you access the calendar. Click on your preferred time (appointments begin on the hour).  Provide any information requested and be sure to confirm the appointment by clicking the “Confirm” button.  You will receive appointment reminders several days before and on the “day of” your appointment.  There is systems help available if you become lost in the process.

 After your initial session, I will ask that you to complete the Gottman Questionnaire which consists of a series of questions designed to pinpoint areas of your relationship that we should focus our initial coaching efforts on.  I will discuss the results of the questionnaire in general terms at your subsequent session.  Your individual responses to the questions will remain confidential and not shared during any session.  Directions on how to access the questions will be provided at your first session. 

This is a secure, HIPPA compliant, website so your personal information will be guarded under the highest standards of client data security.  I do not tape the sessions.  As mentioned in the Disclosure Form, there are only a few situations that client confidentiality will be breached.  I must comply with court ordered disclosure of information.  I am also obligated, by state law to report to appropriate authorities if a client reveals that they intend to hurt themselves or anyone else.  Otherwise, the only reference to your session will be my personal notes and they will be stored in the cloud.  The final stage of the scheduling process will involve online bill paying through a secure web pay provider.  If, for any reason, you decide to cancel or reschedule your appointment, access this website, go to the calendar and delete your name on the current time and (if desired) place your name in another available time slot.

Before your appointment time, you will be sent email with instructions how to install the VSee video conferencing software on your computer or mobile device. At the time of your appointment, come to this website and click on the “Schedule Appointment” button.  This button links you to the VSee subdomain where you can select the “Login” button in the top right corner (by the “Sign up” button) Provide your ID and Password and you will be placed in the online “waiting room”.  From this point I will invite you into my office and we can begin your session. 

Any necessary correspondence will be encrypted and handled over a secure email server address.

If in the case of a natural disaster or computer failure, we can conduct the session over a secure telephone line.  All contact information appears on this website.

Life Guide Center HIPPA Privacy Policy

This notice describes how and what information about you is collected and used. It also describes your rights and how you can get access to that information.

What Information is Collected:
Your information is obtained from the questionnaire you fill out to begin your coaching sessions. The information includes your name, address, telephone number, email address and birthdate and reason for coaching. As coaching sessions proceed, there may be case notes that remind me of details pertinent to your treatment.

How Your Information is Used:
We value your privacy and only use your information to determine the direction and effectiveness of our coaching. We do not share your information with other providers (unless you request this in writing). We do not sell your information to any third party and we do not use your information to do research. We can share your information about you in response to a court or administrative order, or in response to a subpoena. We will share information about you if state or federal laws require it , including with the Department of Health and Human Services if it wants to see that we are complying with federal privacy law. How Your Information is Protected:
We do not store your information at the office. When you submit your information questionnaire or when I take notes, they are stored in a secure cloud location. I must retrieve your information from the cloud before each coaching session. The computer I use, is only used for coaching. It is not used for any internet activity but our coaching session. I use another computer for any internet activity other than your coaching session. The coaching computer is locked in a safe when not used for coaching. The coaching office and the safe are both locked and the only one with access to the keys is the coach himself.

Your Rights:
You have the right to a copy of your coaching information. You may obtain that information by emailing me at joseph@lifeguidecntr.com with your request.

You have a right to a copy of this privacy policy. We recommend that you make a copy of it when you read through it before you schedule your appointment.

Your have a right to complain if you feel we have violated your rights by contacting us at joseph@lifeguidecntr.com If you feel we have not addressed your concerns, you have the right to contact the U.S, Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W. , Washington, D.C. 20201, calling 1-877-696-6775, or visiting www.hhs.gov/ocr/privacy/hipaa/complaints/.

We will not retaliate against you for filing a complaint.

Our Responsibilities:
We are required by law to maintain the privacy and security of your protected health information.

We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.

We must follow the duties and privacy practices described in this notice and give you a copy of it.

We will not use or share information other than as described here unless you tell us we can in writing. If you tell us we can share your information, you may change your mind at any time. Let us know in writing if you change your mind.

Changes to the Terms of this Notice:
We can change the terms of this notice, and the changes will apply to all information we have about you. The changes often result from either change in the laws or regulations or changes in order to clarify our actions and intentions. The new notice will be available upon request, in our office and on our web site.
